Form Initiation

To initiate an Administrative Action form, click on the form name on the main menu. Provide the needed information and click submit. Administrators are given the option to sign off on behalf of instructors but should only do so if the instructor is not available.

Form Action

By selecting "Review and Approve/Deny" under the "Pending Forms" section from the main menu, you can view a list of the forms within your security level that need action. Denying a form will stop all further action; prior approvers and the form initiator will receive an email notice of the denial. Once a form has received all needed approvals, it will route to the Registrar’s Office or Honors College for processing.

Current Status of Forms

By selecting "Current Status and Archive List" from the main menu, you can view the current status of forms within your security level.

Form Search

By selecting the "Search forms " link, you can search for a form based on the Form ID, the Student ID, or the course. From any results, you can see the status of the form and view the form’s details by clicking on the form id.

Cancel a Form

Only forms that are routing or have been approved but not yet processed can be cancelled. Cancelling a form will stop all further routing and processing and allow a new form to be submitted, as needed. Cancelled forms are still visible via the Form Search. To cancel a form, first search for the form using "Form Search". Once found, click on the form id to access the forms details. Scroll to the bottom and click on "Cancel this form?". You must provide a reason for the cancellation.

  • The Instructor should initiate an online Administrative Action for the correction of a grade; see Academic Programs. If the Instructor cannot initiate the action, the Department Chairperson or the Associate Dean of the Course should submit the correction.
  • Administrative Action forms cannot be edited. Forms can be cancelled; a new action can then be initiated.
  • For administrative actions that cannot be completed via this system, there is a hardcopy Administrative Action Form (PDF).
  • If you receive an email message indicating an Administrative Action (Form ID #) is waiting for your review and the form is not in your "Review and Approve/Deny" list, go to the Current Status and Archive List or Search forms based on Form ID to check the status of the form.
